Key Responsibilities- Coordinates the total Plan of Care for patients in the Home Care setting, assessing, planning, implementing, and evaluating patient care.
- Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.
- Coordinates with other healthcare professionals to ensure comprehensive patient care.
- Develops and implements individualized care plans to meet the unique needs of each patient.
- Provides education and support to patients and their families to promote health and wellness.
- Collaborates with other healthcare professionals to ensure seamless transitions of care.
- Graduate of an accredited School of Registered Nursing.
- Current licensure to practice as a Registered Nurse in NYS.
- Valid NYS driver's license.
- One (1) year medical/surgical experience as a RN.
- 1-5 years relevant experience.
- Verbal ability necessary to communicate with patients and others.
- Written communication abilities, to be able to prepare accurate, concise, and meaningful reports.
This is a full-time position with a salary range of $33.45-$47.93 per hour. The successful candidate will be required to work a variety of shifts, including days, evenings, and weekends, as well as participate in on-call and holiday rotations.
