We are seeking a skilled Physical Therapist to join our team at CenterWell Home Health. As a member of our care team, you will play a vital role in helping patients recover from injury, illness, or surgery and regain their independence.
Key Responsibilities- Plan and administer prescribed skilled physical therapy treatment and training for patients with various injuries, illnesses, and functional disabilities.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment and the overall goals of the patient/rehab team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs.
- Provide Physical Therapy Assistants and Home Health Aides with written instructions and care plans that reflect current plans of care.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
