Job Summary

We are seeking a skilled Healthcare Professional to join our team at Della Infotech. As a Travel Nurse, you will have the opportunity to work with a variety of patients and healthcare teams, providing high-quality care and support.

Key Responsibilities
  • Prescribe coordinates and deliver care through the nursing process, maintaining the standards of professional nursing.
  • Supervise CNA and coordinate care with other healthcare workers as assigned.
  • Provide comprehensive initial assessments to all clients and on subsequent visits, continually assessing physical, psychosocial, environmental, and community resources available.
  • Initiate Plan of Care, re-evaluate the need for care, and modify Plan of Care as ordered/needed.
  • Establish discharge plans with patient, physician, other disciplines, and caregivers.
  • Complete OASIS data and provide patient care using current Federal, State, and Agency regulations.
  • Assess educational needs of patient, families, caregivers, and implement appropriate teaching strategies.
  • Complete documentation according to regulatory guidelines.
  • Promote a clean and safe environment of care, utilizing the SAFE error prevention habits.
  • Provide the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct.
Requirements
  • Home Health Case Management experience - strongly preferred.
  • Someone who would go internal after a 13-week contract - preferred.
  • Will consider someone with strong Med Surg experience.
  • OASIS and start of care experience - strongly preferred.
  • Valid drivers license and car insurance.
  • Must be fully COVID vaccinated.
