Nurse Care Coordinator

We are seeking a skilled Nurse Care Coordinator to join our team at Buckner Retirement Services. As a key member of our healthcare team, you will play a vital role in coordinating resident care management and staff direction. Your primary responsibility will be to provide direct patient care that meets or exceeds Buckner quality, service, and hospitality standards.

Key Responsibilities:

  • Ensure that individual health care plans are followed, and administer or supervise treatments prescribed by physicians.
  • Make meaningful rounds of all residents, utilizing health care plans, and make rounds with physicians, accurately and timely recording visits on charts.
  • Administer parenteral, intramuscular, and subcutaneous injections, perform in-house testing procedures, and take and record vital signs.
  • Assist in maintaining current health care plans according to a schedule rotation and documenting acute episodes or significant changes in resident status.
  • Supervise aides in the performance of their duties by monitoring their work, assigning aides to residents, specifying break times, and special duties to be completed.
  • Meet with the director as needed and participate in staff meetings to keep staff performing at an optimum level.
  • Personally receive or place all calls to physicians and follow through with physician's orders timely and accurately.
  • Supervise (or personally handle) the preparation and administration of prescribed medications accurately.
  • Order all new medications and refills, give the initial dose of all new medications, and chart it timely and accurately.
  • Order and maintain nursing supplies, monitor the condition of the medication room, cart, and refrigerator.

Requirements:

  • High School diploma or GED
  • Minimum one-year related experience
  • Current Texas Registered Nurse License
  • Requires sufficient good health to properly discharge duties. Employees shall not be permitted to work who have infectious disease or skin lesion for the duration of the communicability.
  • Requires ability to walk and stand for prolonged periods of time, ability to turn, stoop, kneel, crouch, reach, push, pull, stand, and walk over 75% of the shift.
