Job Description

We're seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Discharge Planner at the Arapahoe County Detention Center. As a key member of our healthcare team, you'll play a vital role in connecting justice-involved individuals to the care they need post-release.

Responsibilities:
  • Coordinate and execute discharge planning services, identifying local resources and arranging appointments with programs or providers.
  • Offer individual and/or group psychoeducational sessions for individuals scheduled for release, fostering their preparedness and support network.
  • Establish and maintain relationships with community agencies serving individuals with mental illness or serious medical conditions, facilitating applications for financial assistance and resumption of benefits.
  • Document services in health records, including community provider contacts, and collaborate with Facility Staff, Institutional Parole Officer, and relevant staff while adhering to security regulations and confidentiality requirements.
Requirements:
  • Graduate from an accredited School of Nursing.
  • Current licensure as a Licensed Practical Nurse (LPN) in the state employed.
  • Maintain CPR certification and any other certifications required by contract.
  • Previous experience in developing successful transition plans involving community resources for persons with medical, substance use, and/or mental illness is preferred.
