Job Title: Licensed Practical Nurse

We are seeking a compassionate and skilled Licensed Practical Nurse to join our team at Hospice of the Western Reserve. As a member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.

Responsibilities:
  • Assess and address the physical, psychosocial, and spiritual needs of patients and their families.
  • Provide skilled nursing services, including pain and symptom management, under the direction of a physician.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Provide emotional support and education to patients and their families.
  • Participate in care conferences and bereavement follow-up.
Requirements:
  • Current licensure as a Licensed Practical Nurse in the State of Ohio.
  • Experience in hospice care, home health, oncology, and/or pain and symptom management preferred.
  • Current CPR certification.
  • Valid driver's license and good driving record.
