Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide personalized care plans to improve patient outcomes.

Responsibilities
  • Conduct adult and geriatric assessments, medical history reviews, and provide diagnosis and treatment plans
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and disease management
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth visits
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills
