Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessments, medication management, and wound care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Conduct regular patient assessments and update care plans as needed.
- Provide education and support to patients and their families on disease management, medication adherence, and healthy living.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Current RN licensure in the applicable state.
- One year of experience in home health or a related field.
- Valid driver's license and reliable transportation.
- Current CPR certification.
