Job Summary

NYC Health + Hospitals/Metropolitan is seeking a skilled Nurse Practitioner to join our team in East Harlem. As a key member of our healthcare team, you will provide high-quality patient care, working collaboratively with physicians and other healthcare professionals to deliver comprehensive health services.

Responsibilities

  • Provide continuous comprehensive medical and nursing care to patients and families, including preventive and supportive health care services.
  • Manage health problems and coordinate health care for various patient populations, ensuring patient-centered quality care.
  • Identify and diagnose illnesses and physical conditions, implementing therapeutic and corrective measures as needed.
  • Develop and maintain partnerships with community-based organizations, making referrals to consulting physicians and/or other healthcare professionals and community agencies as necessary.
  • Evaluate and interpret patient data, planning and implementing appropriate nursing interventions to achieve improved outcomes and patient satisfaction.
  • Consult with physicians and other healthcare practitioners to evaluate health screening findings, making prospective decisions about treatment to support improved outcomes and patient satisfaction.
  • Formulate differential diagnosis and plan of care based on medical history, physical exams, nursing assessment, and other pertinent information.
  • Develop, implement, and evaluate patient plan of care, determining effectiveness and modifying as needed to achieve medical and health goals.
  • Order, conduct, and interpret diagnostic and laboratory tests, identifying and prescribing appropriate pharmacological and non-pharmacological interventions and therapies.
  • Manage patient care as outlined in practice protocols, collaborating with physicians and other healthcare professionals as necessary.
  • Establish medical diagnosis as well as nursing diagnosis as a Certified Nurse Practitioner of New York, in accordance with Article 139, Section 6902.3.
  • Record and document data necessary to maintain and coordinate healthcare, serving as a clinical/educational resource person for nursing personnel.

Requirements

  • A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED).
  • Current certificate as a Nurse Practitioner issued by the NYSED.
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
