Job Summary

Responsibilities:
- Provide direct patient care and manage a caseload of patients with various medical conditions and disabilities
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with our interdisciplinary team to ensure seamless coordination of care and services
- Educate patients and their families on disease management, treatment options, and self-care techniques
- Conduct thorough assessments and evaluations to identify patients' strengths, weaknesses, and areas for improvement
- Lead and participate in multidisciplinary team meetings to discuss patient progress and plan next steps

Requirements:
- Current licensure as a Registered Nurse in New York State or eligibility to practice in the state
- At least two years of experience in med-surg nursing, with a focus on rehabilitation and gerontology
- Strong knowledge of nursing principles, including assessment, planning, implementation, and evaluation
- Excellent communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare professionals
- Ability to prioritize tasks, delegate responsibilities, and manage multiple projects simultaneously
- Familiarity with electronic health records and other clinical software systems
