Job Summary

We are seeking a highly skilled Certified Diabetes Care and Education Specialist to join our team at Lee Health. The successful candidate will be responsible for managing the care of a defined patient population, developing expected patient outcomes, and defining appropriate resource utilization.

Key Responsibilities
  • Provide education and consultation services to internal and external groups
  • Collaborate with other professional staff to facilitate care across the continuum
  • Develop and implement diabetes education programs
  • Conduct individual and group education sessions
  • Facilitate Diabetes Prevention Program (DPP) sessions
Requirements
  • Associate degree in a health-related profession
  • Minimum of 2 years of professional practice experience in Diabetes Self-Management Education
  • CDCES (Certified Diabetes Care and Education Specialist) certification
  • National Registration as an RD (if dietitian) required
  • Current license as an RN in the State of Florida or Florida Licensed Dietitian/Nutritionist
Preferred Qualifications
  • Minimum of 1000 hours in Diabetes Self-Management Education within the past 5 years
