We are seeking a skilled Physical Therapist (PT) to join our Home Health team in New Lenox, IL. As a PT, you will evaluate the physical therapy needs of assigned patients, establish goals, and develop individualized physical therapy programs to improve patient mobility and independent function.

Key Responsibilities
  • Evaluate patient physical therapy needs and develop individualized treatment plans
  • Provide age and developmentally appropriate care to patients of all ages
  • Effectively interact with patients, doctors, and other healthcare members
  • Assess, plan, treat, evaluate, and implement patient care plans
  • Document patient care activities in a timely manner
  • Transfer and position patients as needed
Requirements
  • 1 year of Home Health PT experience
  • Valid driver's license and car insurance
  • COVID vaccination required
  • OASIS and start of care experience preferred
