Position Summary


The Licensed Practical Nurse (LPN) is a certified professional responsible for delivering both direct and indirect care to clients within their residential settings, adhering to the organization's established protocols and guidelines.

The LPN educates, encourages, and supports clients in maintaining their health and preventing illness, thereby enhancing their capacity to live autonomously at home.

The LPN oversees and guides the activities of the Caregiver involved in the client's care plan. Depending on state laws, LPNs may undertake administrative and/or clinical duties.

Key Responsibilities

  • Deliver skilled nursing care/treatment to clients according to Physicians' Orders and the Plan of Care
  • Maintain accurate documentation of nursing interventions, client health status, and communication with families and physicians
  • Record progress and outcomes for set goals, informing the physician, case manager, and families of any changes in the client's medical condition and requirements
  • Administer nursing treatments as prescribed by the physician and within the LPN's scope of practice
  • Manage medication administration as necessary
  • Provide health education to clients, assisting them in mastering appropriate self-care techniques and safety protocols
  • Conduct nursing assessment visits as required
  • Report all incidents in accordance with operational procedures and regulatory standards
  • Adhere to policies and procedures in compliance with operational, state, and regulatory standards
  • Utilize safe body mechanics to prevent injury to self or clients
  • Uphold confidentiality of patient-related information and respect patient rights
  • May be required to cover after-hours, weekends, and holidays on-call to address client needs
  • Complete Start of Care assessments in person, as applicable and in line with state regulations
  • Conduct supervisory visits based on compliance and regulations, including reviewing the plan of care, assessing caregiver competency, and completing necessary documentation. Complete daily, monthly, quarterly, and annual nursing documents as required
  • Observe, assess, plan, implement, and evaluate nursing support for healthy, chronic, or acutely ill individuals in a community or home environment
  • Collaborate closely with the Director of Nursing/Nurse Supervisor and/or Branch Manager in planning, implementing, and evaluating programs and services to meet the health needs of clients
  • Participate in staff meetings, in-services, and other meetings as requested
  • Ensure compliance with state/local/payer requirements, ensuring documentation accurately reflects the care provided and meets company, regulatory, and payer standards
  • Manage the ordering, reviewing, monitoring, discontinuation, and, if necessary, disposal of medications as prescribed by the physician
  • Ensure that policies are in place addressing medication administration, errors, storage/access, and destruction, with annual reviews and revisions as necessary
  • Transcribe or ensure transcription of physician phone orders and other medically related reports
  • Ensure that furnishings, equipment, and supplies are maintained according to guidelines to prevent disease transmission
  • Schedule and complete appointments for health-related issues, implementing appropriate responses to findings in a timely manner
  • Communicate with the Branch Manager, Customer Service Manager, and/or Scheduler regarding client hospitalizations
  • Provide training for new employees, annual re-certification training, and competency validation for health and safety topics
  • May provide case management as required by state regulations
  • Perform other duties as assigned
Qualifications

  • Degree in Nursing from an accredited institution
  • One or more years of nursing experience preferred
  • Experience in Home Health Care preferred
  • Proficient in medication administration
  • Current and unrestricted registration and license in the state of practice
  • CPR professional certification
  • Basic computer skills and the ability to learn industry-specific software and documentation applications
  • Strong oral, written, and interpersonal communication skills with all levels of personnel, both internal and external
  • Excellent organizational and time management skills with a keen attention to detail
  • Ability to handle highly confidential information responsibly
  • Capable of working effectively both independently and as part of a team
  • Strong critical thinking skills, capable of researching situations, solving complex problems, and addressing a variety of issues
  • Willingness to learn and achieve various educational and position-specific goals
  • Meet all pre-employment requirements as per company and/or state regulations
  • Basic clinical skills
