Job Title: Home Health Physical Therapy Assistant

LHC Group is seeking a skilled Home Health Physical Therapy Assistant to join our team in Paterson, New Jersey.

About the Role:

As a Home Health Physical Therapy Assistant, you will play a vital role in providing high-quality care to patients in their homes. Your primary responsibility will be to assist the supervising Physical Therapist in developing and implementing treatment plans that improve the patient's well-being and functional abilities.

Key Responsibilities:
  • Assist the supervising Physical Therapist in conducting patient evaluations and developing treatment plans.
  • Provide exercise instruction and patient education to promote active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
  • Observe, record, and report to the supervising Physical Therapist, nurse supervisor, and/or physician the patient's response to treatment and changes in the patient's condition.
  • Coordinate care with other members of the healthcare team as appropriate.
  • Instruct patients, family, caregivers, and other members of the healthcare team in areas of therapy within the scope of the physical therapy assistant.
Requirements:
  • Current licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
