Key Responsibilities- Implement the physician's prescribed plan of care to ensure the patient's health and well-being
- Conduct patient assessments and coordinate care with the healthcare team
- Administer prescribed medication, treatments, and therapies as ordered by the physician
- Document care provided in a timely and compliant manner
- Provide health education and training to family members to promote patient independence
- Communicate effectively with the healthcare team, including physicians, case managers, and other caregivers
- Active, unencumbered LPN license in the state of practice
- Current CPR certification from a recognized organization
- TB skin test (current within the last 12 months)
- At least six months of prior hands-on nursing experience (preferred but not required)
- Reliable transportation
