Job RequirementsWe are seeking a skilled Home Care Physical Therapist to join our team at Liberty Health. The ideal candidate will have a bachelor's degree in physical therapy from an accredited school and current licensure from the state where therapy will be provided.
Salary Range: $75,000 - $110,000 per year
Responsibilities:
- Provide patient care services along with direct treatment and follow-up.
- Evaluate patients new to service where there are physician orders and develop a plan of care appropriate to the patient's diagnosis.
- Document patient care data accurately, thoroughly, and in a timely manner.
- Communicate with intercompany and intracompany contacts as necessary.
