About the Role

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Provide Home-Based Care: Deliver comprehensive care to patients with various medical conditions, injuries, and chronic diseases, often after discharge from a hospital, rehabilitation, or nursing facility.
  • Collaborate with Healthcare Professionals: Work with physicians and oversee LPNs, CNAs, and aides to provide patient care, ensuring seamless coordination and communication.
  • Assess and Document Patient Progress: Conduct thorough examinations, document patient progress, and report changes to physicians, maintaining accurate and detailed records.
  • Education and Coaching: Educate patients on their plan of care, prescribed medication, therapy, diet, and exercise, as well as coach family members on patient care, medication, and home safety.
  • Discharge Planning: Ensure goals are met and coordinate discharge from services, ensuring a smooth transition to the next level of care.
Requirements
  • Active RN License in Colorado: Possess an active Registered Nurse (RN) license in the state of Colorado.
  • Home Health Experience: While not required, prior home health care experience is beneficial.
  • No Case Manager Responsibilities: This role does not involve case management responsibilities.
  • Graduate of an Accredited School of Nursing: Be a graduate of an accredited school of nursing.
  • At Least 1200 Hours of Experience as an RN: Possess at least 1200 hours of experience as a Registered Nurse.
  • Healthcare CPR/BLS Certification: Hold current Healthcare CPR/BLS certification, with the ability to demonstrate the course.
  • Background Check and Drug Test: Pass a background check and drug test, including THC testing.
  • Travel Requirements: Be willing to travel to various locations, including Greeley, Fort Collins, Loveland, and surrounding areas.
