Nurse Specialist

Job Summary:

This is a full-time opportunity for a skilled RN to join our team as a Nurse Specialist. You will be responsible for assessing patient needs, developing individualized care plans, and coordinating with interdisciplinary teams to ensure seamless care transitions.

Responsibilities:

• Conduct thorough assessments of patients' medical, emotional, and social needs
• Develop and implement personalized care plans that address patient goals and priorities
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
• Provide education and support to patients and families on disease management, medication regimens, and self-care techniques
• Document patient progress and communicate effectively with the care team to ensure continuity of care

Requirements:

• Graduate of a school of nursing
• Active Washington State Registered Nurse License
• Provider BLS - American Heart Association within 30 days of hire
• 2 years of Nursing experience preferred
• 1 year of Hospice, home health, skilled nursing facility, medical/surgical, or oncology experience preferred
