Key Responsibilities- Provide direct nursing care to critically ill patients in accordance with established policies, procedures, and protocols of the healthcare organization.
- Implement and monitor patient care plans, ensuring that all aspects of patient care are met.
- Serve as a primary coordinator of all disciplines for well-coordinated patient care, working closely with our interdisciplinary team to ensure seamless care transitions.
- Note and carry out physician and nursing orders, ensuring that all necessary actions are taken to meet patient needs.
- Assess and coordinate patient's discharge planning needs with members of the healthcare team, ensuring a smooth transition to post-acute care.
- Licensed Registered Nurse credentialed from the Wisconsin Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
- Graduate of a board-approved Registered Nurse Program required.
- Bachelor of Science in Nursing (BSN) preferred.
- ~ 1 year nursing experience, BSN, ACLS and stroke certified.
