Job Summary:

The Registered Nurse Weekend will be responsible for providing comprehensive care to patients in their homes, including LVN and HHA supervision, case conferences, and discharge planning. This role requires strong communication and documentation skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Supervise and manage all aspects of patient care, including LVN and HHA supervision
  • Coordinate the distribution of visits among all disciplines (SN, PT, OT, ST, MSW, HHA) after the Start of Care, Recertification or Resumption of Care is completed
  • Lead weekly interdisciplinary case conferences for patient caseload, and contribute to the development of care plans for patients
  • Coordinate all aspects of discharge planning with patients, clinical team, and agency
  • Provide individual and group help for patients/residents and families at times of adjustment, crisis, or particular need
  • Review and enforce TheraCare Home Health Clinical Policies and Procedures on Abuse Prevention
  • Participate in on-call weekend rotation, including PRN staff, and work a minimum of one major and one minor holiday each year
Requirements:
  • Ability to read, write, and speak the English language
  • Strong communication and documentation skills
  • Ability to work independently and as part of a team
  • Knowledge of TheraCare Home Health Clinical Policies and Procedures on Abuse Prevention
