Case Management Services Director

The Customized Assistance Services is seeking a highly skilled and experienced Case Management Services Director to administer, coordinate, and monitor SUPPM Case Management (CM) Programs. The ideal candidate will have a strong background in case management, social services, and program administration.
The Case Management Services Director will be responsible for:
- Administering, coordinating, and monitoring ORS Programs to provide comprehensive services for adult populations designated by HRA.
- Reviewing specific troublesome case issues with ORS vendors and other HRA offices to maintain services for the subject population and improve service delivery.
- Offering creativity and innovation in the design and development of policies and programs that increase the self-sufficiency, employability, and wellness of clients with disabilities.
- Ensuring timely completion of substance-use assessments through the Substance Use Centralized Assessment Program (SUCAP).
- Working closely with other parts of the agency to ensure effective communication and coordination of care for CAS clients.
- Developing and implementing standard operating procedures and reporting mechanisms for monitoring the effectiveness and timeliness of ORS contractors/providers.
- Facilitating operational meetings involving ORS providers and relevant HRA departments to enhance the exchange of information and ensure effective service delivery.
- Producing reports to evaluate the program's effectiveness in meeting its goals while ensuring a high quality of service to clients.
Requirements:
- Baccalaureate degree from an accredited college or university and four years of progressively responsible experience, including one year at the administrative or managerial level in a large governmental agency, business firm, civic or community organization operating in the area of social services.
- Strong administrative and management skills.
- Subject matter expertise in at least one and preferably a number of the following areas: health, mental health, substance use disorders, vocational rehabilitation, case management.
- Experience working with underserved populations such as substance users, veterans, homeless individuals, ex-offenders, and LGBTQI.
- Experience designing and implementing policies and programs that increase self-sufficiency, employability, and wellness of clients with disabilities.
- Experience working with contracted human services programs.
- Ability to work across agency and City organizational lines within multidisciplinary initiatives.
- Excellent writing and presentation skills.
