Job Description:
This role involves coordinating care through a collaborative care model, directing patient care, and ensuring patient rights are observed and followed. The successful candidate will also attend in-service training programs, staff meetings, and provider contract negations.
Responsibilities- Coordinate care through a collaborative care model
- Direct patient care
- Assure that patient rights are observed and followed.
- Participate in in-service training programs, staff meetings, and provider contract negations.
- Collaborate with fellow clinicians to ensure effective and timely healthcare delivery.
- Identify equipment and supply needs of the clinic.
- Ensure continuity of care by providing services at local non-Association of Clinicians for the Underserved sites.
- Deliver contracted services on behalf of Association of Clinicians for the Underserved.
- Foster effective working relationships and contribute to achieving organizational goals.
- Achieve specific goals and objectives within established timeframes.
