Key Responsibilities
  • Develop and Implement Care Plans: Create personalized care plans through physician orders, client input, and nursing assessments to achieve measurable goals and outcomes.
  • Patient Education and Support: Educate clients and their family members on specific needs, promoting independence and self-care.
  • Primary Caregiver Training: Properly orient and train primary caregivers to ensure optimal functioning levels for each client.
  • Continuum of Care Coordination: Coordinate hands-on client care through documentation and timely communication with physicians and other caregivers.
  • Home Health Care Services: Provide care in patients' homes using various skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
Requirements
  • Active RN License: Possess an active RN license in the state of application.
  • CPR Certification: Hold valid CPR certification.
Preferred Qualifications
  • Medicare Skilled Nursing Experience: Possess experience in Medicare skilled nursing.
  • Basic Understanding of Oasis: Have a basic understanding of Oasis.
  • 1-Year RN Experience: Possess at least 1 year of RN experience in a healthcare setting.
