Join EAC Network as a Part-Time Case Manager

We are seeking a dedicated and compassionate individual to join our team as a Part-Time Case Manager for our Alternatives for Youth (AFY) Program. As a Case Manager, you will provide vital support to families in crisis, helping them find stability and avoid the need for legal interventions.

Key Responsibilities:
  • Assist families in crisis by working closely with them to resolve issues that led them to seek help through a PINS petition.
  • Provide intensive case management, conducting home-based visits, assessing needs using the Child and Adult Needs Assessment (CANS), and creating personalized Family Intervention Plans.
  • Offer crisis intervention to guide families toward stability and make referrals to partner agencies and find new resources when necessary.
  • Teach behavior management strategies to parents and youth to foster a healthy home environment.
  • Stay organized by maintaining detailed case records and progress notes, ensuring everything is updated in the system on time.
  • Be on call to respond to families reaching out via the AFY hotline during scheduled times.
  • Provide transportation for clients when needed for emergency resources like clothing or medication.
  • Lead workshops and participate in youth events to provide life skills and guidance.
Requirements:
  • Bachelor's Degree with at least one year of experience working with children and adolescents, or at least one year in community/social service work.
  • Valid Driver's License and own vehicle for client visits and transport.
  • Comfortable using Microsoft Office, email, and databases to manage case records.
  • Participate in 20 hours of training during your first 6 months, with another 20 hours in the next 6 months.
