Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality care to clients in their homes.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to work with adult and geriatric clients in Ocean County. As a member of our multidisciplinary healthcare team, you will be responsible for developing and implementing individualized care plans, conducting diagnostic tests and measurements, and educating clients and their families on rehabilitative care and activities.

Responsibilities:
  • Conduct home visits to clients in designated geographic territories.
  • Perform diagnostic tests and measurements, such as mobility and range of motion assessments.
  • Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
  • Continually assess and revise care plans, and participate in multidisciplinary care planning as needed.
  • Educate clients and their families on rehabilitative care and activities.
Qualifications:
  • A current New Jersey Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
