We are seeking a highly skilled Patient Care Coordinator to join our team at St. Tammany Parish Hospital. As a Patient Care Coordinator, you will be responsible for ensuring the effective delivery of patient care by monitoring and evaluating patient care processes and outcomes.
Key Responsibilities- Monitor and evaluate patient care processes and outcomes to ensure high-quality care.
- Coordinate daily scheduling of patient visits and ensure appropriate staffing levels.
- Participate and lead interdisciplinary care teams to ensure seamless patient care.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Ensure compliance with regulatory requirements and hospital policies.
- Current LA RN licensure, BSN/Master's preferred.
- Current BLS certifications through any International Liaison Committee on Resuscitation (ILCOR) provided courses.
- Three years of clinical experience.
- One year of Home Health or Hospice experience.
- Valid and current state-issued driver's license.
- Clearance of driving record check.
- One year of management experience preferred but not required.
- Knowledgeable of ICD-10 code guidelines preferred.
The Patient Care Coordinator must possess good physical health and be able to perform the following tasks:
- Standing, sitting, or walking for long periods of time.
- Lifting at least 50 pounds.
- Working with a moderate level of noise.
