LPN Home Health Professional Opportunity

Providence is seeking a skilled LPN to join our Home Health team. As a Licensed Practical Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.



About the Role

This is a full-time LPN position in our Home Health department, providing medical care and support to patients in their homes. You will work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring our patients receive the best possible care.



Responsibilities


  • Provide skilled nursing care to patients in their homes, including assessments, wound care, and medication management.
  • Develop and implement individualized care plans in collaboration with our interdisciplinary team.
  • Conduct regular home visits to monitor patient progress and adjust care plans as needed.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive patient care.
  • Document patient information, including assessments, care plans, and progress notes, in a timely and accurate manner.
  • Maintain a safe and healthy work environment, adhering to all relevant laws, regulations, and Providence policies.


Requirements


  • Current licensure as an LPN in the state of Oregon (or Washington, if practicing in this state).
  • Graduate of an accredited LPN/LVN school of nursing.
  • One year of experience in nursing, preferably in home health, hospice, or long-term care.
  • National Provider BLS certification, obtained within 30 days of hire.
  • Valid driver's license and reliable vehicle, with proof of insurance.

Job Requirements


  • Pay range: $27.58 - $43.01 per hour.
  • Full-time position, 40 hours per week.
  • Day shift, with occasional evening and weekend hours.
  • Must be able to travel to patient homes in the Hillsboro, Gaston, Forest Grove, and Cornelius areas.


Please note that the pay range listed is the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
