Patient Care Specialist

As a Patient Care Specialist at Luminis Health, you will contribute to delivering high-quality, cost-effective healthcare by providing direct and indirect patient care, collaborating effectively with the healthcare team, and participating in quality improvement initiatives. This role requires a competent member of the healthcare team who can apply clinical nursing knowledge and skills in a specialization unit.
Key Responsibilities- Demonstrate clinical nursing knowledge and skill in the specialization of the unit.
- Apply the nursing process effectively in the care of culturally diverse patients and families.
- Organize and reprioritize patient care activities based on subtle and overt environmental changes.
You will consistently and thoroughly assess patients to collect data and identify learning needs according to established standards and policies. Your systematic analysis of assessment data will help develop individualized problem lists for assigned patients. You will also develop and implement plans of care in accordance with established standards, appropriate prioritization of problems/needs, and mutually agreed-upon goals.
Your expertise in medication administration, pain management, and other unit-specific skills will enable you to efficiently implement patient plans of care. Continuous evaluation of plan effectiveness will inform revisions and recommendations based on patient responses to interventions.
In addition to direct patient care, you will participate in QI, C PI, and risk management activities at the unit, department, or organizational level. Your ability to effectively perform and improve processes will contribute to achieving excellence in AAMC's quality standards and benchmarks.
You will also participate in unit shared governance, education, and orientation of new staff, delegation of patient care activities, and real-time computer documentation when completing patient records.
Requirements- Graduate of an accredited school of nursing.
- Adherence to credentialing requirements of AAMC as stated in the nursing bylaws.
- Current licensure as a registered nurse by the Maryland Board of Nursing.
- American Heart Association CPR for Healthcare Providers.
