We offer a comprehensive range of medical and chronic care services, backed by the expertise of SCAN, a not-for-profit organization dedicated to improving the health and independence of older adults for over 45 years.
We are expanding our services into the Philadelphia/Center City area and seeking part-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our team.
Responsibilities:- Provide primary care in the home setting, Board and Care, and Assisted Living Facilities
- Drive to patient locations in your assigned geographic area
- Average 7-10 patient visits per day (if working a full workday)
- Exhibit exceptional bedside manner and communicate complex medical information with simplicity and kindness
- Provide health education to patients and work with them to prevent illness, promote health, and manage chronic health problems
- Utilize our EMR system to document patient visit/interaction accurately and timely
- Knowledge of HCC coding
- Active participation in an on-call rotation is required
- Other duties as assigned
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required
- Current, unrestricted PA or NP license to practice in Pennsylvania including DEA license
- Minimum of 1 year of experience in primary care, hospice, or urgent care/outpatient setting
- Previous experience in a home health, hospice or other home visit setting is highly desired
- Enrolled in Medicare preferred
- Keystone credentialed preferred
- PA Medicaid enrollment preferred
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding
- Strong computer skills including MS Office and Outlook is strongly desired
- Great verbal and written communication skills
- Current drivers' license, auto insurance and reliable transportation
- Spanish speakers are strongly preferred
- Compassionate and caring attitude with a desire to live our mission, vision and values
- $85.00 per hour
