Hackensack Meridian Health is seeking a highly skilled and dedicated Ambulatory Pharmacist to join our team. As a key member of our healthcare organization, you will play a vital role in providing exceptional patient care and contributing to our mission to transform healthcare and serve as a leader of positive change.
Key Responsibilities- Comply with all state and federal regulations and Medical Center policies and procedures relating to drug dispensing, including appropriate handling of drug products, adherence to security procedures, and completion of necessary documentation.
- Review and dispense prescriptions, clarifying any potential problems with prescribers.
- Monitor patient drug therapy regimens for clinical appropriateness, including issues such as allergies, drug-drug interactions, drug-food interactions, potential adverse reactions, appropriate dosing, and compliance.
- Perform assigned unit and pharmacy inspections to ensure proper maintenance and storage of medications, supplies, including emergency drug supplies, controlled substances, and hazardous drugs.
- Oversee activities of ancillary personnel, including Pharmacy Technicians, Liaisons, Navigators, Clerks, and Couriers.
- Employ principles of safe medication practice as dictated by departmental and organizational policies and procedures.
- Lift a minimum of 10 lbs., push and pull a minimum of 40 lbs., and stand a minimum of 4 hours a day.
- Identify the needs of the patient population served and modify and deliver care that is specific to those needs, including communicating with patients, parents, and/or primary caregivers at their level.
- Perform other duties and/or projects as assigned.
- Graduate of an ACPE accredited school of pharmacy, or equivalent, with a minimum of a Bachelor's Degree in Pharmacy.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Microsoft Office and/or Google Suite platforms.
- A minimum of one year of pharmacy experience.
- Registered Pharmacist License.
