We are seeking a highly skilled and experienced Primary Care Physician to lead our newest Senior Primary Care Office in Jacksonville, Florida. As a key member of our team, you will be responsible for providing high-quality patient care, leading a multidisciplinary team, and contributing to the growth and development of our organization.
Key Responsibilities- Provide exceptional patient care, addressing medical needs and social determinants of health
- Lead a multidisciplinary team, including Advanced Practitioners and Care Team members
- Develop and implement care plans, ensuring patient-centered and coordinated care
- Collaborate with other healthcare professionals to improve patient outcomes and quality of care
- Participate in daily huddles and contribute to the development of care protocols and policies
- Doctor of Medicine or Osteopathic Medicine degree
- Board Certification/Board Eligibility in Family, Internal, or Geriatric Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license
- Two to five years of directly applicable experience
- Experience managing Medicare Advantage panels and understanding of Best Practice in coordinated care environments
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Guaranteed base salary + 20% bi-annual quality incentive
