Home Health Speech Language Pathologist Per Diem Opportunities

Become an integral part of our dedicated care team and help us put health first in the lives of our patients. As a Home Health Speech Language Pathologist, you will play a vital role in delivering comprehensive speech language pathology services to individuals in need of home-based care.
Job Summary:Evaluate, direct, and provide speech/language pathology services to patients in their homes or facilities, while working closely with healthcare professionals to develop and implement individualized care plans. Utilize your expertise to assess and treat disorders of speech, voice, language, hearing, and swallowing, ensuring the highest quality of life for our patients.
Key Responsibilities:- Evaluate patients' speech and language abilities, identifying areas of strength and weakness to inform treatment plans.
- Develop and implement personalized care plans, collaborating with multidisciplinary teams to ensure seamless care coordination.
- Administer and interpret diagnostic tests, including audio logic screenings, to inform treatment decisions.
- Provide education and training to patients and their families on alternative communication systems and strategies.
- Maintain accurate records of patient progress, communicating effectively with healthcare teams to ensure continuity of care.
- Meet the educational and experience requirements for Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
- Possess at least six months of experience as a speech therapist/speech-language pathologist, with home health experience highly desirable.
- Hold a current and unrestricted license to practice as a speech-language pathologist in your state.
- Be certified in CPR and maintain valid driver's license and auto insurance.
We offer a competitive salary range of $63,400 - $87,400 per year, commensurate with experience, along with opportunities for career growth and professional development.
