Job Summary

The Private Duty Nurse RN will be responsible for implementing the physician's prescribed plan of care, conducting patient assessments, and coordinating care with other healthcare professionals. The ideal candidate will have excellent communication skills, a strong commitment to patient-centered care, and the ability to work independently in a fast-paced environment.

Responsibilities
  • Implement the physician's prescribed plan of care, including administering medications, treatments, and therapies as ordered.
  • Conduct patient assessments and coordinate care with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
  • Provide education and training to patients and their families on disease management, medication adherence, and other relevant topics.
  • Document all care provided in a timely and accurate manner, ensuring compliance with regulatory requirements and company policies.
  • Collaborate with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
Requirements
  • Active, unencumbered RN license in the state of practice.
  • Current CPR certification from the American Red Cross or American Heart Association.
  • TB skin test (current within the last 12 months).
  • At least six months of prior hands-on nursing experience, preferably in a home health or hospice setting.
  • Reliable transportation and a valid driver's license.
