About the Role

  • We're seeking a skilled Registered Nurse with 1 year of experience in an acute inpatient setting to join our team. In this role, you'll provide clinical direction for all aspects of patient care, fostering patient-centered care and demonstrating clinical competency in evidence-based practices and research.
  • You'll work collaboratively with providers and other members of the healthcare team to ensure seamless care coordination, identify realistic and measurable goals and outcomes, and develop individualized plans of care that meet the unique needs of each patient.
  • In addition to your clinical expertise, you'll possess excellent communication and interpersonal skills, allowing you to effectively educate patients and families about their care plans and advocate on their behalf.
  • As a Nurse Care Coordinator, you'll be responsible for collecting data, completing nursing assessments, and evaluating the effectiveness of patient care plans.
  • Your attention to detail and ability to prioritize tasks will enable you to manage multiple patients' needs simultaneously, ensuring timely and high-quality care.
  • You'll also participate in performance improvement initiatives, identifying opportunities for quality enhancement and implementing evidence-based best practices to improve patient outcomes.

Requirements

  • A Associate degree in Nursing (Bachelor's degree preferred)
  • 1 year of experience as a Registered Nurse in an acute inpatient setting (or completion of the Prisma Health Nurse Residency Program or comparable residency)
  • Licensed to practice as a Registered Nurse in South Carolina

Estimated Salary: $65,000 - $90,000 per year
