About the Role:
- Provide high-quality, patient-centered care in the comfort of patients' homes
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Conduct thorough assessments, identify needs, and coordinate care with other healthcare professionals
- Monitor patient progress, adjust care plans as needed, and communicate effectively with patients and families
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
Requirements:
- Current, unrestricted state license as a Registered Nurse in the state of practice
- Current CPR certification
- Experience in home healthcare or a related field (med-surg, new grad, or new to home health welcome)
- Ability to travel and visit adult patients in their homes
