Kaiser Permanente is in search of a dedicated Registered Nurse (RN) specializing in Case Management. This role involves managing patient care effectively while ensuring compliance with healthcare regulations.
Key Responsibilities:
1. Evaluate patient needs and recommend alternative care levels when inpatient hospitalization is not medically necessary.
2. Act as an indirect caregiver and patient advocate, managing care in a cost-effective manner without compromising quality.
3. Collaborate with physicians and the multidisciplinary healthcare team to develop and document individualized care plans.
4. Assess high-risk patients requiring post-hospital care planning and coordinate discharge plans to meet their needs.
5. Monitor and evaluate patient hospital stays to ensure timely delivery of essential services.
6. Facilitate continuity of care through effective utilization management and transfer coordination.
7. Conduct clinical reviews for quality management activities across various healthcare settings.
8. Serve as a liaison between inpatient facilities and referral agencies, providing comprehensive case management.
9. Educate healthcare team members on their roles in the discharge planning process and resource utilization.
10. Provide patients with education to assist with their discharge and address psychological challenges related to their health conditions.
Qualifications:
- Minimum of two years of combined RN experience in acute care or case management.
- Completion of an accredited RN training program.
- Strong understanding of utilization review, discharge planning, and case management principles.
- Knowledge of regulatory requirements and accreditation standards.
- Excellent planning, organizational, conflict resolution, and negotiation skills.
- Proficient in computer literacy.
- Bachelor's degree in nursing or a related healthcare field.
Work Schedule:
- Flexibility in shift scheduling, including potential weekend and holiday coverage.
