Key Responsibilities- Patient Care:
- Assist with daily living activities, including bathing, dressing, feeding, and mobility support, ensuring residents' comfort and well-being.
- Medication Administration:
- Safely administer prescribed medications and treatments, monitor for adverse effects, and document appropriately.
- Vital Signs and Monitoring:
- Regularly monitor and record residents' vital signs, report any changes in health status to RNs and physicians, and assist with ongoing care plans.
- Documentation:
- Maintain accurate and timely documentation of resident care, including charts, medication logs, and care reports in compliance with regulatory standards.
- Care Coordination:
- Collaborate with Registered Nurses (RNs) and other healthcare professionals to execute personalized care plans for residents, ensuring all medical and emotional needs are met.
- Resident Advocacy:
- Act as an advocate for residents' rights, ensuring they receive respectful, compassionate, and appropriate care.
- Infection Control and Safety:
- Follow infection control procedures and ensure a clean, safe, and sanitary environment for residents and staff.
- Communication:
- Communicate effectively with residents, families, and healthcare team members regarding care updates, changes in condition, and any concerns.
- Supervision:
- Provide guidance and support to Certified Nursing Assistants (CNAs), ensuring they meet residents' needs and follow facility protocols.
- Emergency Response:
- Respond promptly and appropriately to emergencies, following facility protocols for patient safety and wellbeing.
- Current Licensed Practical Nurse (LPN) license in Pennsylvania or compact license
- Graduate of an accredited LPN program
- Minimum of 2 years of nursing experience
- Current CPR/BLS certification
- Strong clinical skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a fast-paced healthcare environment
- Compassionate and patient-centered approach to care
- Familiarity with electronic health record (EHR) systems
- Shift: 5x8 Hr. Nights (40 Hrs. per Week) (11:00pm - 7:30am)
- Duration: 9 Weeks
