Pharmacist PRN Job Description

We are seeking a highly skilled and motivated Pharmacist PRN to join our team at ResCare Premier Inc.

Responsibilities:
  • Evaluate physician medication orders for appropriateness, including drug, dosage, potential interactions, and route of administration.
  • Make recommendations for therapy changes to physicians and facility staff as needed.
  • Maintain patient/resident databases with current and past medical history, clinical and laboratory data, and concurrent medication.
  • Ensure laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaborate with the healthcare team to provide clinical and technical support.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for safe drug procurement, storage, distribution, and use.
  • Obtain and maintain required documentation in accordance with federal and state regulatory agencies and policy.
  • Inform the Pharmacy Manager of medication-related errors.
  • Assist facility staff with procedures related to safe and effective drug methods.
Qualifications:
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications as required by government regulations and business needs.
