Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Great Lakes Mgmt Co. As a key member of our healthcare staff, you will be responsible for ensuring the health and well-being of our residents in a dynamic and supportive environment.

Key Responsibilities
  • Conduct thorough assessments of potential residents to ensure their needs are met.
  • Develop and implement individualized care plans to promote optimal health and quality of life.
  • Collaborate with interdisciplinary teams to coordinate resident services and ensure seamless care transitions.
  • Monitor and maintain accurate and up-to-date resident records, including medical histories and treatment plans.
  • Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making and optimal care outcomes.
  • Supervise and train staff members on medication administration, medical procedures, and resident care protocols.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure regulatory compliance.
  • Provide education and training to staff members on resident care, medical procedures, and infection control practices.
  • Collaborate with physicians and other healthcare professionals to ensure timely and effective care.
  • Participate in on-call rotations to provide 24/7 coverage and support.
Requirements
  • Current state licensure as a Registered Nurse.
  • Minimum one year of experience in home health, assisted living, or long-term care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and decision-making skills.
  • Experience working with seniors and their families.
  • Ability to maintain confidentiality and handle sensitive information.
