Responsibilities
- Implement physicians' prescribed plans of care for patients with complex medical needs
- Conduct thorough assessments of patients' conditions and coordinate care with healthcare teams
- Administer prescribed medications, treatments, and therapies as directed by physicians
- Document patient care and progress accurately and timely in our electronic system
- Provide education and training to family members on patient care and medication management
- Collaborate with healthcare teams to address patient concerns and changes in condition
- Perform other duties as assigned by supervisors or management
Requirements
- Active, unencumbered license (RN/LPN/LVN) in the state where you will practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Six months prior hands-on nursing experience preferred but not required
- Reliable transportation to visit patients in their homes
