Job Overview

Northern Lights Health LLC is seeking an experienced Primary Care Nurse Practitioner to join our team. As a key member of our interdisciplinary healthcare team, you will be responsible for providing high-quality primary care and wound care services to residents/patients in Transitional Care Units (TCUs) and Long-Term Care Units (LTCs).

About the Position

The ideal candidate will have a Master's or Doctoral degree in Nursing (NP), current state Nurse Practitioner license, certification as a Nurse Practitioner in Adult-Gerontology, Family, or another relevant specialty. Previous experience in primary care and wound care is essential, as well as strong clinical and assessment skills, excellent communication and interpersonal skills, ability to work collaboratively with a diverse healthcare team, proficiency in electronic health records (EHR) systems, and compassion and dedication to delivering patient-centered care.

Responsibilities:
  1. Provide Comprehensive Primary Care Services: Diagnose, treat, and manage chronic and acute medical conditions, monitor medication regimens, and provide patient education on preventive care, lifestyle modifications, and self-management of chronic conditions.
  2. Specialize in Wound Care: Assess, manage, and treat wound-related conditions, utilizing evidence-based practices to promote wound healing and minimize complications.
  3. Collaborate with Interdisciplinary Team: Work closely with the healthcare team to ensure seamless care coordination and a holistic approach to patient care.
  4. Develop Individualized Care Plans: Create and implement care plans addressing unique needs and preferences of each resident/patient, focusing on improving overall health and well-being.
  5. Maintain Accurate Medical Records: Ensure compliance with all regulatory requirements and facility policies by maintaining up-to-date medical records.
