UnitedHealth Group is seeking a highly skilled Nurse Practitioner to join our HouseCalls team. This dynamic role offers an excellent opportunity for a dedicated healthcare professional to make a meaningful impact in the lives of patients across Texas and Louisiana.

As a member of our Home and Community Care team, you will be responsible for conducting annual clinical assessments in patients' homes, identifying health risks, and coordinating follow-up care to improve their overall well-being. Our integrated medical teams are redefining healthcare by providing high-end medical services, compassionate care, and industry-leading solutions to vulnerable patient populations.

With this role, you will have the flexibility to achieve a great work-life balance. Our flexible scheduling allows providers to choose from a range of shifts across seven days a week, without requiring on-call hours, weekends, or holidays. Additionally, all travel expenses are covered, and you will receive a competitive base salary plus multiple incentives.

Your primary responsibilities will include:

  • Conducting in-home assessments, including past medical history evaluations, medication reconciliations, vital signs checks, and comprehensive physical exams
  • Performing evidence-based practice screenings, including point-of-care testing as necessary
  • Identifying diagnoses for care management and active medical management
  • Formulating lists of current and past medical conditions using clinical knowledge and judgment
  • Communicating assessment findings with members' primary care physicians to inform potential gaps in care
  • Recognizing urgent and emergent situations and intervening accordingly
  • Educating members on disease processes, medications, and compliance

To succeed in this role, you must possess the following qualifications:

  • Active NP license or ability to obtain one by start date
  • Licensure must be unencumbered and free of any open/unresolved disciplinary actions
  • APCs working in jurisdictions that authorize autonomous practice must have obtained approval to practice autonomously or without formal supervision
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification or ability to obtain it by start date
  • Ability to complete physical requirements of the job, including lifting 30-pound bags and navigating various dwelling conditions
  • Access to reliable transportation for up to 85% of time within licensed states

Preferred qualifications include:

  • 1+ years of clinical experience in highest level of education, clinical setting
  • Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
  • Home Health care or home visit experience
  • May be requested to obtain additional licensure in other geographic areas

Estimated Salary: $95,000 - $125,000 per year, based on experience and location.
