At CenterWell Home Health, we're dedicated to delivering personalized care to patients in the comfort of their own homes. As a Home Health Speech Language Pathologist, you'll play a vital role in helping patients recover from illness, injury, or surgery, and regain their independence.

Key Responsibilities:
  • Evaluate and provide speech-language pathology services to patients in their homes or facilities
  • Develop and implement individualized plans of care to address patients' unique needs
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Provide education and training to patients and their families on effective communication strategies and techniques
  • Conduct regular assessments and evaluations to monitor patients' progress and adjust treatment plans as needed
Requirements:
  • Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
  • Minimum of six months experience as a speech therapist/speech language pathologist
  • Home health experience a plus
  • Current and unrestricted license
  • Current CPR certification
  • Good organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation
