Rush University Medical Center is seeking a highly skilled Clinical Pharmacy Specialist to join our team. As a key member of our pharmacy department, you will be responsible for providing clinical pharmacy services to patients, including order verification, discharge counseling, and education of healthcare professionals.
Responsibilities- Evaluate and verify Epic physician's orders for therapeutic appropriateness, proper dosing, and optimal dosage form.
- Clarify physicians' orders when necessary through direct and timely contact with prescriber or designated practitioner.
- Provide patient-specific pharmacotherapy recommendations and drug information to medical and nursing staff.
- Participate in the pharmacy department's documentation of clinical interventions.
- Perform daily review of pharmacy reports, including pharmacokinetic monitoring and other drug or patient-specific reports.
- Conduct monthly nursing unit inspections of medication storage areas.
- Supervise pharmacy technician staff involved in medication preparation and delivery.
- Coordinate medication ordering, preparation, dispensing, and administration systems.
- Implement drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.
- Participate in SE (ADR and medication error) reporting programs.
- Complete clinical and operational activities as designated by the departmental management team.
- Precept pharmacy students and residents in clinical and operational areas.
- Assess and evaluate medical and drug literature to compare and contrast therapeutic regimens.
- Implement medication safety initiatives as determined by current departmental needs.
- Develop operational and clinical projects as determined by current departmental needs.
- Serve as a liaison with other departments for collaborative research activity.
- Evaluate and monitor drug therapy relative to the total needs/problems of the ambulatory care patient.
- Actively participate in professional organizations.
- Bachelor of Science in Pharmacy.
- Illinois Pharmacist License.
- Minimum of four years of Clinical Pharmacy experience or PGY 1 residency completed within the last seven years.
- Knowledge in intravenous admixture services, intravenous drug therapy, and chemotherapy.
- Ability to work effectively with others to provide safe and effective drug therapy.
- Excellent problem-solving skills and ability to work independently and quickly in responding to and resolving a wide range of problems.
