Clinical Social Worker

We are seeking a seasoned Clinical Social Worker to join our team in the Emergency Department at LifeBridge Health. As a key member of our interdisciplinary team, you will provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk patients presenting to the Emergency Department.

In this critical role, you will collaborate with clinical teams and medical providers to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources among a patient population with complex health needs.

• Provide patient and family advocacy, discharge planning coordination, and psychosocial intervention in the Emergency Department
• Collaborate with clinical teams and medical providers to promote patient and family wellness and improve care outcomes
• Develop and implement individualized plans to address social determinants of health and promote patient self-management
• Establish relationships with community resources and healthcare providers to ensure seamless transitions and continued support for patients

• Masters in Social Work (MSW) or equivalent degree
• MD LMSW license required
• 1-3 years of experience in social work or related fields

• Competitive salary range: $28.00-$49.00 per hour, based on experience
