Key Responsibilities:
- Assess patients' physical and functional capabilities to create tailored care plans.
- Design, execute, and adjust physical therapy treatment programs based on medical referrals and evaluations.
- Administer hands-on therapy, exercises, and techniques to enhance mobility, alleviate pain, and restore functionality.
- Educate patients and their families on therapeutic exercises, proper body mechanics, and strategies for injury prevention.
- Collaborate with interdisciplinary healthcare teams to ensure comprehensive and coordinated patient care.
- Maintain precise and timely documentation of patient evaluations, progress notes, and treatment plans.
- Engage in continuous professional development to remain informed about current trends and best practices in physical therapy.
Qualifications:
- Graduate of an accredited School of Physical Therapy.
- Current State license as a Physical Therapist or a multi-state license issued by a state member of the National Physical Therapy Compact.
- A minimum of two years of experience as a Physical Therapist in a clinical care environment, with a preference for experience in Home Health, geriatrics, or medical-surgical settings.
- Exceptional interpersonal and communication skills.
- Strong nursing skills as defined by generally accepted standards of practice.
- Reliable transportation with valid and current auto liability insurance.
- Ability to work flexible hours and travel locally as required.
- Capability to lift up to 50 lbs. and perform physical tasks associated with patient care.
