Job Summary

PMHCC, Inc. is seeking a highly skilled Community Based Nurse to join our team. As a Community Based Nurse, you will be responsible for providing healthcare supports, training, and technical assistance to individuals diagnosed with intellectual/developmental disabilities (I/DD).

Key Responsibilities
  • Nursing Services: Provide clinical consultation for individuals with complex health conditions, act as a resource regarding medical/dental treatment, care, and follow-up, complete nursing reviews, and obtain certification in developmental disabilities nursing certification (CDDN).
  • Training and Education: Provide training in physical and behavioral health care issues for individuals diagnosed with I/DD, staff members, family members, supports coordinators, and other supportive personnel.
  • Technical Assistance: Provide technical assistance around PCHC initiatives, such as Behavioral Health Team Review of Psychotropic Medication form, Health Promotion Activity Plans (HPAPs), and Dysphagia project.
Requirements
  • Registered Nurse/Valid Pennsylvania Nursing License
  • Two years of experience working with individuals with I/DD (or other acceptable nursing experiences)
  • Experience in providing training in health care issues
