Job SummaryWe are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team as a Home Health LVN/LPN. As a Home Health LVN/LPN, you will play a vital role in providing patient-centered care to our patients in the comfort of their own homes.
Responsibilities- Provide skilled nursing care to patients in their homes, as prescribed by physicians and in accordance with established care plans.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Administer medications and treatments as ordered by physicians, and document patient responses and progress.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Provide education and support to patients and their families on disease management, medication use, and other health-related topics.
- Document patient information, including progress notes, medication lists, and other relevant data, in accordance with company policies and regulatory requirements.
- Graduate of an accredited practical or vocational nursing program.
- Currently licensed as a Practical Nurse or Vocational Nurse in the state of agency operation.
- Valid CPR certification.
- Ability to travel to all business locations, including patient homes and company offices.
