The Health Center at Dallas Retirement Village is a 121-bed rehabilitation and long-term care establishment dedicated to providing exceptional care in a supportive and collaborative atmosphere.

The LPN is responsible for overseeing daily nursing operations conducted by nursing assistants and delivering direct nursing care to residents. The LPN guarantees that the highest standard of care is provided to residents in accordance with current federal, state, local, and facility regulations, as well as directives from the Registered Nurse Manager.

Key Responsibilities:
  • Provides and manages care for residents within a designated unit, including conducting admission assessments, transfers, and discharges.
  • Administers medications and treatments, processes orders, documents in clinical records, assists physicians, and supervises nursing staff in line with established protocols.
  • Reviews medication orders and administration records for completeness, accuracy, and compliance with policies.
  • Assigns employee duties and has the authority to adjust responsibilities as necessary to meet resident needs.
  • Conducts visits to assigned residents to assess their physical and emotional well-being during shifts, notifying the RNM and physician of any changes.
  • Provides direct care as needed, including taking vital signs and assisting with resident mobility.
  • Initiates and updates resident care plans based on assessed needs, including completing necessary documentation.
  • Communicates resident status and updates to the RNM and other relevant parties through written and verbal reports.
  • Implements facility policies and procedures consistently and fairly.
  • Evaluates nursing staff performance and provides guidance on job performance and quality of care.
  • Informs family members or responsible parties of any changes in resident conditions.
  • Completes incident reports for residents and staff as required.
  • Maintains confidentiality of all care-related information to protect resident rights.
  • Engages with residents and their families to discuss care plans and address concerns.
  • Participates in ongoing education and supervisory meetings regularly.
  • Handles telephone inquiries professionally.
  • Orders necessary medications, supplies, and equipment following facility protocols.
  • Addresses complaints and proposes solutions to resolve issues.
Qualifications:
  • Must possess a valid, unrestricted LPN or RN license in the State of Oregon.
  • Current BLS CPR certification is preferred.
  • Ability to interact tactfully with staff, residents, families, visitors, and the public.
  • Knowledgeable in nursing and medical practices and procedures.
  • Demonstrates supervisory skills, effective communication, and the ability to collaborate with both professional and non-professional staff.
  • Capable of planning, organizing, and scheduling tasks for assigned staff while interpreting care objectives and policies.
  • Highly customer service-oriented with a commitment to providing excellent hospitality and care.
