MDS Director

Evergreen Center for Health & Rehabilitation is seeking a dedicated and detail-oriented MDS Director to lead our Minimum Data Set (MDS) program. As an MDS Director, you will be responsible for ensuring compliance with state and federal regulations, managing the MDS program, and driving continuous improvement in nursing care. Key Responsibilities:

* Lead and oversee the MDS program to ensure compliance and accuracy
* Train, mentor, and manage MDS Coordinators
* Collaborate with interdisciplinary teams for accurate data collection
* Track, trend, and analyze assessment data for compliance and optimization
* Drive a culture of continuous improvement and innovation in nursing care

Qualifications:

* Valid state RN license preferred
* Direct care in a long-term care setting and/or MDS Coordinator experience required
* Proficient in state and federal regulations governing the MDS and billing process
* Excellent written and verbal communication and interpersonal abilities
